What to know for your first visit
We're looking forward to welcoming you to ORA Dental.
Please arrive 10 minutes early
to complete any paperwork if you haven't already filled in your new patient form online.
New patient form
Once your appointment is confirmed, we'll send you a link to complete our new patient form in advance, this helps us make the most of your time at your first visit.
Children
Please bring along anything that helps your child feel comfortable, a favourite toy or comfort item is always welcome.
What to bring:
Your NHI number, if known
Details of any medical conditions, medications, or allergies
Your ACC details, if your visit relates to an accident
Insurance details, if applicable
Parking
There is plenty of free street parking available at the Milldale Neighbourhood Centre.
